If you're a manager and want to recruit to fill a vacancy, set up a new post, or change a contract or authorise an additional payment, please refer to the terms of reference and complete the form below, as appropriate, to raise the request for discussion at the workforce board.
This form is to be completed by the service/team manager by close of play on the Friday before the board meets.
NB: before starting to complete this form, you'll need the following information:
- Service area
- Responsible assistant director/service director
- Request type (additional hours/overtime payment; agency worker; contract amendment; honorarium; job evaluation; market supplement; recruitment, conference/training/expenses)
- If the request is for an honorarium or job evaluation you'll need to detail the proposed salary point
- If the request is recruitment-related, please state whether it can be done as an FTC and/or redeployment/apprenticeship
- Post number (as listed in ERP). Where there's no post number (eg new post, additional resource agency staff) use P00000 and provide further detail under 'rationale'
- Post title (as listed in ERP)
- Position hour
- Cost of request
- Detail of any transformation project the request is linked to
- Cost centre to be used, and confirmation that there's enough budget to cover any cost AND whether projections already cover the cost or will it lead to a higher projected spend. If there's no budget, please add details of where this is being funded from (external funding, etc)
- Rationale for the request. Brief description/detail of request (add info not already included in the form). Reason for request/risks if not approved, etc
Once the form has been submitted, a copy of the submission will be emailed to you.
The relevant assistant director/service director will then be asked to review and recommend (approve/decline/defer) to the board. If approved the request will be sent to the workforce board for consideration and decision.
The relevant assistant director/service director will advise you of the board’s decision.
